Answers to the most asked hair transplant questions — graft survival, when results show, shaving requirements, donor area limits, and how to choose a clinic in Istanbul.

Planning a hair transplant in Istanbul? These 18 questions cover everything from technique selection to realistic expectations. Based on real patient consultations and post-op feedback.
Technique & Procedure Questions
**What's the difference between FUE and DHI?** FUE: follicles extracted with micro-punch → channels created with blade → grafts placed with forceps (two separate steps). DHI: follicles extracted the same way → loaded into Choi pen → pen creates channel and places graft simultaneously. DHI offers more angle control for hairlines; FUE handles larger sessions faster.
See our detailed FUE vs DHI comparison. **What is Sapphire FUE?** Standard FUE uses steel blades to create recipient channels. Sapphire FUE uses sapphire-tipped blades that create smaller, smoother incisions — faster healing and slightly denser packing.
It's an enhancement to FUE, not a separate technique. **How many grafts do I need?** Norwood 2-3 (receding hairline): 1,500-2,500 grafts. Norwood 3-4 (hairline + mid-scalp): 2,500-3,500 grafts. Norwood 4-5 (hairline + crown): 3,500-5,000 grafts. Norwood 6+ (extensive): 5,000-6,000+ grafts (may need 2 sessions).
Your surgeon will assess via photo consultation. **Does the surgeon or a technician do the procedure?** Critical question. In many Istanbul clinics, technicians perform the extraction and implantation while the surgeon supervises. In surgeon-led clinics, the surgeon personally performs the critical steps.
Ask specifically: "Will the surgeon extract and implant, or will technicians?" Price is higher for surgeon-led (~€3,000-€6,000 vs. €1,500-€2,500 for technician-led).

Results, Recovery & Timeline
**When will I see results?** Month 1-2: transplanted hair falls out (shock loss — normal and expected). Month 3-4: new growth begins as thin, fine hairs. Month 6-8: significant visible density improvement. Month 10-14: final result. Full assessment at 12-14 months. **Will the transplanted hair fall out again?** No.
Transplanted follicles are taken from the permanent zone (back and sides of head) which is genetically resistant to DHT (the hormone causing male pattern baldness). These follicles retain their permanent characteristic in their new location. **Do I need to shave my head?** For standard FUE: usually yes (both donor and recipient areas). For unshaven DHI: no — surgery is performed between existing hairs.
For partial-shave: only donor area is shaved (can be covered by longer hair above). Discuss shaving preference during consultation. **How painful is it?** Local anesthesia makes the procedure painless during surgery. The injection itself is the most uncomfortable part (5-10 minutes). Post-op: mild tenderness for 2-3 days, sleeping on an elevated pillow for 7 nights. Most patients return to desk work in 2-3 days.

Donor Area & Limitations
**Is there a limit to how many grafts can be taken?** Yes. The average donor area supports 6,000-8,000 grafts total across all sessions. Taking too many grafts (over-harvesting) thins the donor area permanently, creating visible gaps at the back.
A responsible surgeon will decline to extract beyond safe limits. **Can body hair be used as donor?** Beard hair (chest, beard) can supplement scalp donor in some cases, but body hair has different texture, growth cycle, and survival rate.
It's a backup option, not a primary source. **Will the donor area look normal after?** With responsible extraction (no more than 25-35% of follicles per cm²), the donor area heals completely — micro-dot scars are invisible at 3mm+ hair length. Over-harvested donor areas show visible moth-eaten thinning. **Can I have multiple sessions?** Yes, but limited by donor capacity.
Most patients can safely have 2-3 sessions with 12+ months between each. Plan your total graft need upfront so the surgeon can distribute donor resources across sessions. **What about hairline design?** This is arguably the most important decision. An unnatural hairline (too straight, too low, too aggressive) cannot be undone. Review 50+ before/after photos with hairlines similar to what you want.
Ask the surgeon to draw the proposed hairline before surgery and approve it. Get your personalized assessment.
Frequently asked questions
Can women get hair transplants?
Yes. Female pattern hair loss, traction alopecia, and hairline lowering are all treatable with FUE/DHI. Women typically don't shave the full head — partial or no-shave techniques are used.
Does hair transplant work for alopecia areata?
Not recommended. Alopecia areata is an autoimmune condition that can attack transplanted hair. Hair transplant works for androgenetic alopecia (genetic pattern baldness), not autoimmune hair loss.
Should I use finasteride after transplant?
Often recommended to protect existing (non-transplanted) hair from further loss. Transplanted hair is permanent regardless. Discuss finasteride and minoxidil with your surgeon — side effects exist and not everyone needs them.
How much does a hair transplant cost in Istanbul?
FUE: €1,500-€3,500. DHI: €2,000-€4,500. Sapphire FUE: €1,800-€4,000. Surgeon-led premiums are 30-50% higher. See our full cost guide.
